Web Form referer problem

Please note the following if you get an error when submitting this form.

Because of a new kind of junk e-mail technique, we have been forced to turn on additional security protections in this form. If you get an error message when submitting this form that refers to a problem with "bad referer" you are probably submitting this form from an organization that has certain protective features enabled for web browsing.

In that case, please try using this form at your home, where this is not likely to be a problem.

As an alternative, try using this form at another location which is not part of the same organization. Note, however, that many organizations use this protection feature, so you may get the same error again. You are more likely to experience this problem at medium to large organizations, and also at ones exceptionally concerned about security, regardless of size.

If you experience this problem even from your home, please call us!

We apologize for the inconvenience.

Selecting Multiple Items

This brief chart explains how to select more than one thing in form fields that present multiple, pre-defined choices as a list. This does NOT apply to check boxes, radio boxes, or any field where you type in the information. Instructions are provided for both Windows and MacOS based PCs.

Windows

MacOS

Select one choice

Left-mouse-click on desired item.

Mouse-click on desired item.

Select range (i.e. everything from one choice up to and including another choice)

Left-mouse-click on first item.
<SHIFT>left-mouse-click on last item.

Mouse-click on first item.
<SHIFT>mouse-click on last item.

Select assorted (i.e. more than one item not in a range)

<CTRL>left-mouse-click on any item to either choose it or un-choose it!
Repeat until all desired items are selected.

<CONTROL>mouse-click on any item to either choose it or remove it!
Repeat until all desired items are selected.

Select range with exceptions and/or additions (i.e. more than one item not in a range, or supplement above)

Use select range procedure first, then use select assorted procedure to add choices, remove choices, etc. You can even un-choose items previously chosen within a range.

Notes:

In a particular form selection box you can only use <SHIFT> to select one range. If you need two separate ranges, save time by doing the longer one with <SHIFT>, and then use <CTRL> or <CONTROL> to add the others individually.

Doing the select range procedure un-selects all items outside the range! Therefore, always do the range procedure first, followed by the additional selections.

If you click a choice without holding down any modifying key, all previous choices are forgotten, and that becomes the one and only choice. Be careful to not wipe out your effort!

You can hold down the <CTRL> or <CONTROL> key while doing a series of choices; you don't need to let it go and press it again for each choice!

You can always use <CTRL> or <CONTROL> to select or un-select choices, whether or not you used <SHIFT>.
