Join us for an open-hearted discussion about using the nationally recognized Five Wishes Document, one of the most popular living wills in the U.S., for advance care planning.

The document is designed to help individuals define end-of-life wishes regarding treatment guidelines and how/whom to appoint as Health Care Agent in case of a situation when they cannot speak for themselves.

DOROT social workers will provide context, and documents. This workshop this is valuable for anyone considering their own options and doing future planning, as well as people helping with loved ones.
